Set up ACH Payment to Comcast using customer form

I work for a government county agency. We pay our account (beginning with 9) by check at the moment and it takes a long time to reach you by US mail. The only way our county auditors will allow a supplier/vendor like Comcast to be set up for ACH payments is to complete the county's ACH form. This is to prevent fraud. 

1. Will you complete our county form?

2. If yes, to whom or where would I submit this form?

We would normally not complete any external forms like that on our end for customers over this platform. You would have to make sure to get that submitted and if you need any additional assistance we are here to help.
