$800 Prepaid Visa Gift Card not sent after 9 months

on my contract it says I was due a $800 visa prepaid gift card after 30 days with a 6-9 week delivery time.

i started service in March, it is now January of the new year and I still haven’t received it.

the business fulfillment site says “authorized” and has said authorized for months. When is this going to be sent? Customer service has not been helpful in receiving this.
