My modem works for about 12 minutes at a time then loses connectivity and has to be reboot which takes another 10-15 get get back online.

Thanks for troubleshooting your service and for the additional information. I am seeing that your modem that operates internet service is still offline. We may need to schedule a tech visit to resolve the issue. I can see the modem that operates phone service is online. Do these modems share a splitter by chance or do they connect to separate cable outlets?
