Suddenly an equipment fee..why?

I have suddenly been hit with a user equipement fee when I have been a customer at a contracted price for over 4 years...there is no optuion to get a billing error corrected in ANY menu or to contact billing via meail..what is going on and how can I get this fixed without staying on hold for 2 hours????
