Disconnect my service

I have been trying for WEEKS to reach your "retention team".  They do not answer your phone option 5 and I have been told I would get a callback and I have not got one. 

 

This serves as notice to disconnect my account as of March 1, 2015.  Account number xxxx, service address xxxx.  If you need to reach me or need the equipment returned please email xxxx.
