Account closed - no reason why

We have an account at one of our locations in Bellevue, WA that had a MDTA unit and several cable cards.  For some reason the service was cancelled in Feb of this year, but no one seems to be able to tell me why the service was cancelled.  I have a hard time believing that the service was cancelled and no one took note of the person putting in the request.  I need some assistance in finding out how this happened.
