constant outages cant do business!!

Our TV, internet, and phone is constantly going out.  Third time in a week.  It has been out for about 6 hours.  We can't get the work done as all communication is cut off!  Who can I complain too? Caller ID incorrect despite numerous calls

After modem changed I only get 25 Mpbs down but I pay for 50 Mbps
