Overpriced services. Contract termination required.

Dear Sir or Madam,
Due to upward price changes, the inflated cost of your services and our inability to pay these large bills, we would like to close the account.
Please, send us a document to sign the termination of the contract /permanently disconnection form to my e-mail. Thank you.
