new account, being charged for a modem I don't have

My bill is showing a 9.99 charge for equiptment rental, but I am providing my own modem... how do I get this charge removed?  I don't see any method for contacting billing anywhere.

The equipment charge has been removed off of your account. This adjustment will show up on you next bill.

I have this same problem. I have supplied my own modem/wireless gateway, but I'm being charged a monthly equipment fee. Can you please remove this charge? Thanks.
