Connectivity lost around 4:30 pm EST on 7/2

Power was lost in my area on 6/29 due to the recent derecho. It was restored on 6/30 and I had no issues with network connectivity until 7/2 around 4:30 PM. Since then the SMC lights have been displayed as follows:

Power: solid green
DS: blinking green
US: off
ONLINE: off
LINK: solid green
DIAG: off

Under the SMC's web interface in Gateway Summary > Cable Modem > Initialization Procedure, Initilize Hardware is "Process...". Also the Downstream Channel section appears to be locking in 3 out of the 4 channels, and then loosing lock after the display automatically refreshes.
