Can't port toll free numbers out of Comcast

I have two toll-free numbers that I have been attempting to port from Comcast to Anveo (my new carrier). I made the initial request mid-November. Comcast has rejected the porting request twice due to an "incorrect signature", but I signed the porting authorization form, and I am the account owner.

 

I need to get my two toll-free numbers ported out of Comcast ASAP.
